Westwood Post-Acute is a CMS 5-Star Rated skilled nursing facility and one of the largest post-acute care centers in the Bay Area. We're looking for a dedicated Physical Therapist to help residents regain mobility, improve independence, and achieve their rehabilitation goals.

What You'll Do

• Evaluate residents and develop individualized treatment plans
• Provide high-quality physical therapy services
• Collaborate with an interdisciplinary healthcare team
• Educate residents, families, and staff on treatment goals and progress
• Maintain accurate documentation and EMR records
• Promote safe, effective, and resident-centered care

Qualifications

• Graduate of an accredited Physical Therapy program
• Active California Physical Therapist license
• Current BLS/CPR certification
• Strong communication and teamwork skills
• Experience in skilled nursing or long-term care preferred
